The integrated care program for discharged patients is designed to reduce the gap in health management that patients experience when they return home without additional care after completing hospital treatment. Concerns have consistently been raised that linkage between hospitals and local welfare systems is crucial, particularly for elderly people or people with disabilities who live alone and have limited mobility, as a lack of appropriate care following discharge carries a high risk of leading to readmission.
The meeting was organized based on this awareness of the issue, bringing together the district and local medical institutions to discuss a system for sharing information on discharged patients and procedures for linking them with care services.
Through this cooperative system with medical institutions, the district plans to identify residents who need care after discharge at an early stage and quickly connect them with the various care services operated by the Senior and Disabled Welfare Division.
A district official said the meeting is expected to further strengthen cooperation between the medical and welfare sectors.
